Plasmacytoma (Multiple Myeloma) Extramedullary plasmacytoma of testis is rare, with fewer than 50 reported circumstances; it may symbolize an early manifestation of a quantity of myeloma or may be found at autopsy of sufferers with multiple myeloma. Only a couple of primary testicular tumors with no proof of bone marrow involvement have been reported. Pathologic Features the gross look is that of a pink-tan, lobulated, firm, ill-defined, or discrete nodule. The tumor may be a fleshy mass up to 8 cm, and customarily no hemorrhage, necrosis, or cyst formation is seen. A predominantly interstitial infiltrative growth sample of immature plasma cells is seen, with frequent invasion of seminiferous tubules, blood vessels, and tunica albuginea. The tumor cell population is homogeneous and consists of cytologically immature plasma cells. Rare tumors include neuroblastoma, Wilms tumor, retinoblastoma, mesothelioma, and Merkel cell tumor. High magnification shows predominantly interstitial localization of metastatic tumor with relative sparing of seminiferous tubules. Ulbright and Young416 reviewed their expertise with 26 circumstances from consultation (n = 23) or their hospital (n = 3) information; not one of the instances was from post-mortem materials or incidentally discovered in therapeutic orchiectomies from sufferers with prostate most cancers. The tumors occurred in males 29 to 90 years old, with the prostate being the commonest major web site (n = 11), adopted by the renal parenchyma (n = 4), colon (n = 4), urinary tract (n = 3), lung (n = 2), esophagus (n = 1), and, likely, small gut (carcinoid, n = 1). Noteworthy findings included the frequent absence of a recognized primary tumor (62%), the rarity of bilateral involvement (8%), the occasional lack of a definite mass on gross examination (15%), the infrequency of multinodularity both grossly (8%) or microscopically (35%), the prominence of intertubular growth (42%), conspicuous intrarete or intratubular progress in some cases (especially prostate carcinoma) (19%), prominent cytoplasmic vacuoles in occasional instances (15%), and the frequent presence of lymphatic involvement (69%). Four tumors (three prostate, one renal) with distinguished intrarete and/or intratubular development had submitting diagnoses of either a primary rete neoplasm or seminoma.
